In fact, there are more bosses with the prefix "moons" in the game, but the four listed are optional. They can only be summoned if you have a moon scale!
From the Valley of Despair, the altar near which you fought the Second Prince and the King of the Flowing Sands, enter the cave. Return to the Wolf Forest from the first chapter, to the Forest altar (outside). Go forward, to where you fought the Wandering Wight. On the opposite side from the undead (if moving from the altar, then in the far left corner) look for a pond with a waterfall. Interact with the waterfall to stop the water and go to a secret location with the Red Moon.
In the second chapter, go to the Stone Plain altar near the place where you fought the Stone Guardian. In the far part of this area there are steps. Climb them. There will be a blue spearman here, when killed, the spirit of the spearman drops. Behind him is a waterfall of sand. Interact with it to stop the sand (if there are moon scales). Go to the secret area with the Black Moon.
In the third chapter, having got on the back of a huge turtle, from the Turtle Island altar go down to the edge of the turtle, where a statue of a small moon is visible. Come closer, and your hero will call the Blue Moon (if there are scales).
If you could activate and kill the previous three moons in any order, then Yellow Moon appears only after Red, Black and Blue are killed!
Move to the Oblivion Rock altar in the Web Pit, follow the path to the left of the altar, through the cave. In this place, you fought the Right Hand of Buddha. Go over the huge insect, jump down a couple of times until you get to a fork. You need to jump to the left, to a rope bridge. Cross it, follow through the cave and run along the bridge without sides (you can hide from enemies in cocoons using Misty Step). Eventually, you will go outside, to a green location. Climb a little higher to activate the Dead Moon's Ravine altar.
Climb even higher, and Yellow Moon will be standing in the gazebo. If it's not here, it means you haven't killed one of the three moons described above. After defeating the Yellow Moon, you'll receive the Golden Carp oddity, which increases the chance of materials dropping from enemies you kill.
