Thorm Mausoleum is located in the far northeast corner of the Shadow-Cursed Lands. Follow the House of Healing and the dense graveyard next to it. You will need the help of Astarion and Shadowheart.
Raphael will be waiting for you near the Tomb. Talk to him. Explore the tomb.

For clues in the next area, take the temple map from the table in the north room that separates the first coffin from the second, open one.
