Up to this point (that is, right now), you can change the gender of the character exclusively on a paid basis, for 800 rubles. The feature remains available through hairdressing games, and has been around for over a decade. John Hight, the game's executive producer, commented on this point, stating that during the development of the expansion, many creators came to the conclusion that paying for a gender change was the wrong move.
The release of the add-on itself is scheduled for the fall of this year, and beta testing will begin next week.
